Hello everyone, this is an official announcement from the representative of YG FAMILY FanClub.

Fan of overseas who satisfies the requirements for 2NE1 OFFICIAL BLACK JACK 3rd

(Live in Korea, Have a certificate of alien registration) have to pre-apply

with “collecting private information consent form” a set period.

This notice is guide for pre-application, please send a mail a set period

after the form will attached this notice.

Please read the following.

◆ Guide for preparation of collection private information consent form ◆

(1) Foreigner standards

 - Live in Korea, Have a certificate of alien registration

(2) How to prepare

Print “collecting private information consent form” after the download.

-> ① write the collecting private information consent form your own handwriting

-> ② prepare the certificate of alien registration which must be a valid date.

-> Scan ①collecting private information consent form and ②certificate of alien registration

   You should send a mail to 21blackjack@ygmail.net after the scan completely

   (apply period will fixed later)

(3) Preparation document

(4) Notice for writing of collecting private information consent form

① Collecting private information consent form must be written your own handwriting.

② Receipt is impossible if there is even one a blank on collecting private information consent form.

③ Have to scan that two documents are prepare completely. (take a picture X)

④ You cannot register to BLACK JACK 3rd if you write the bogus information or forge a document.

⑤ Certificate of alien registration has not expired,

   Only if you renew your certification is BLACK JACK activities possible later

   if your certificate of alien registration is close to expiration.

   a certificate of alien registration in advance.

Cannot register application for BLACK JACK 3rd

if foreigner(Live in Korea, Have a certificate of alien registration) is not submitting pre-application.

2NE1 OFFICIAL BLACK JACK 3期加入を希望する海外ファンの皆様の中

BLACK JACK 3期に加入可能な条件（国内に居住し、外国人登録証を所持）に

該当する方々は、決まった期間内に“個人情報収集同義書”様式をダウンロードして

事前加入受付をしてください。

本告知は事前案内です。追後様式が添付された後、受付期間にお申し込んでください。

詳しい事項は下記をご確認ください。

◆ 個人情報収集同義書準備方法 ◆

(1)　外国人会員基準

国内に居住し、外国人登録証をお持ちの方

(2)　準備方法

個人情報収集同義書をダウンロードしてプリント

-> ①　本人直筆で個人情報収集同義書作成

-> ②　有効期間が過ぎてない外国人登録証をご用意

-> ①　作成した個人情報収集同義書と②外国人登録証をスキャン

　スキャンまで全て完了した後21blackjack@ygmail.net へメール申し込みをしてください。

　（受付期間別途確認）

(3)　準備文書

①　本人直筆で作成した個人情報収集同義書

②　外国人登録証

①番文書と②番文書をスキャンしたファイル

(4) 個人情報収集同義書作成時注意事項

① 必ず本人が直接直筆で作成してください。

② 個人情報収集同義書に空いている所が一つでもあった場合受付が不可能です。

③ 準備完了した受付書類2個は必ずスキャンしてください。（写真撮影 X)

④ 個人情報収集同義書虚偽で作成したり、偽造した場合、 BLACK JACK 3期に加入出来ません。

⑤ 外国人登録証は有効期間が過ぎてない物で、

   満了日が残り少ない場合、個別的に更新してください。満了日が過ぎた場合BLACK JACK活動が不可能です。

◆ 個人情報収集同義書受付方法 ◆

1．受付対象：国内に居住し、外国人登録証をお持ちの海外ファン

2．受付方法: 21blackjack@ygmail.net メールを通じて受付

　　加入を希望する海外ファンの方々は上記の内容を宿志して

　　予め外国人登録証をご用意してください。

事前加入受付をしない外国ファンの方々は追後加入申請書を作成出来ませんので

必ずこの点含んでおいて受付期間内にお申し込んでください。

ファンの皆様のたくさんの関心と声援お願いします。

想加入第三届BLACK JACK的海外粉丝当中满足加入条件（国内居住，外国人注册证书持股人）的人，清下载“个人信息同意书”的方式后，在有效期内，一定做事先加入接受。

本公告事项是事先介绍，方式附加以后，清接受期间内接受。

◆ 个人信息同意书准备方法◆

(1) 外国人会员基准

-国内居住，外国人注册证书持股人。

(2) 准备文件

下载个人信息同意书后输出。

-> ① 用本人亲笔填写个人信息同意书。

-> ② 准备没经过的有效期的外国人注册证书。

-> ①作完的人信息同意书和②外国人注册证书扫描。

   清扫描完后伊妹儿接收在 21blackjack@ygmail.net（清真实接受期间）

(3) 准备文件

① 本人亲笔写的个人信息同意书。 

②外国人注册证书。

(4) 个人信息同意书填写时注意事项

① 一定本人亲手亲笔填写。

② 如果有空格在个人信息同意书的话不可以接受。

③ 准备玩了得两件文件一定要扫描。（拍照不行）

④ 如果虚伪填写个人信息同意书后者伪造的话，不能加入第三届BLACK JACK。

⑤ 外国人注册证书应该是不经过的，如果期满不久的话应该刷新才能加入BLACK JACK。

◆个人信息同意书接受方式◆

1.接受对象：国内居住，有外国人注册证书的海外粉丝。

2.接受方式：伊妹儿接收在21blackjack@ygmail.net

清记住上端的内容，想加入的海外粉丝，清准备外国人注册证书。

没有事先加入接受的海外粉丝们以后不能填写加入申请书，清记住一下一定要接受期间以内接受。
